What this document has to state
- Exactly what is being contracted: the assessment, the protocol, and what falls outside the published price.
- The cancellation and refund policy, with deadlines.
- Jurisdiction and applicable law, bearing in mind that a good share of patients are foreign nationals.
In the meantime, no form on this site sends anything. We do not collect health information without first being able to tell you how it is handled.
